New Patient Information
What you need to know before your first appointment with us.

Please download the form at the bottom of this page and complete prior to your visit.

You will need to bring your new patient registration form, insurance card, and a photo ID so that we may get a copy of these for our records.

Payment is expected at time of service. Co-Payments and/or deductibles will be collected prior to seeing the provider. We will file covered charges to your primary insurance. We do not file secondary insurance.

We accept most major credit/debit cards and cash. We do NOT accept checks.

We do not fill out permanent disability forms. Short term disability forms can be completed only if our providers take you out of work and are subject to a fee.

We will not reschedule initial evaluation appointments if you do not show up or call 24 hours in advance to cancel.

Please have any referral or office notes from other providers you would like our doctors to have faxed prior to your appointment on our secure fax line 864-877-5684.

*From I85 North, take the Pelham Road Exit, Exit 54. At the top of the exit ramp, turn left onto Pelham Road. From Pelham Road, turn right onto The Parkway. At the third light, turn right onto Elmshorn. After about 0.3 miles, turn right onto Parkway Commons Way. Our office is number 68.
*If coming from I85 South, simply turn right onto Pelham road and follow the same directions.
